COVID-19 declaration form information for RIW cardholders

This article has been created to assist RIW cardholders with information regarding COVID-19 declaration forms. 


This instruction applies to the following RIW System User Roles: 

  • myRIW - RIW Cardholder


At some worksites COVID-19 status will be assessed using the RIW System to hold COVID-19 status for RIW cardholders. These worksites will be responsible for verifying the cardholder’s COVID-19 declaration form, sighting the evidence of COVID-19 status and ensuring compliance to NSW and VIC state health directives and RIW privacy policies and procedures. 


Cardholders will be required to fill in a COVID-19 declaration form and provide the supporting documentation for sighting to the worksite’s Verification Officer. This is to ensure that no sensitive medical data is held as evidence for the employer based competency.


  1. After reading the state based (NSW or VIC) declaration form the RIW cardholder will be required to complete the declaration, and provide evidence of their status for sighting to the Verification Officer. 
  2. The RIW cardholder will be required to agree to the COVID-19 Declaration by reading and signing the bottom of the form.  
  3. The completed COVID-19 Declaration form will then be added to the RIW cardholder profile as an Employer based competency.

NOTE: Only the declaration form will be provided as evidence against the Employer Based competency in the RIW system.

Creating COVID-19 status as Employer based competencies as an Employer Administrator with Premium functionality.

This article has been created to provide guidelines on how Premium Employers can use Employer based competencies to demonstrate COVID-19 status for RIW cardholders. 


This instruction applies to the following RIW System User Roles:

  • Helpdesk
  • Network Manager
  • Network Manager - Read Only
  • Employer Administrator (Premium Functionality Employer)
  • Employer Administrator (Premium Functionality Employer) - No Payment
  • Employer Administrator (Premium Functionality Employer) - Read Only
  • Advanced Project Administrator
  • Project Administrator
  • Site Administrator
  • myRIW - RIW Cardholder


Premium Employer will be able to create new employer-based competencies, within the RIW system for recording COVID-19 status for RIW cardholders. 


Naming Convention example, in VIC: JH – COVID19 – 1st Vaccination, JH – COVID19 – 2nd Vaccination, JH – COVID19 – Medical Exempt.


The health directives can be different for each state so it is the worksites responsibility to ensure that they are granting access in accordance with the relevant states requirements. 


To demonstrate their COVID-19 status RIW cardholders will be required to fill in the declaration form for the relevant state (NSW or VIC), depending on the worksite they are accessing, and then provide evidence of their COVID-19 status for sighting to the Verification Officer for the worksite.


The Verification Officer will verify the documentation supplied and sign the declaration to confirm. Only the declaration will be uploaded as evidence for the Employer based Competency held within the RIW cardholder’s profile.

Verifying COVID-19 status as a Verification Officer with Premium functionality.

This article has been created to assist with providing guidelines to worksite representatives actions as Verification Officer to sight COVID-19 evidnece presented by RIW cardholders.

The Verification Officer is a worksite representative that is nominated to perform verification duties associated with confirming the COVID-19 vaccination, testing or exemption status nominated on the relevant state COVID-19 declaration form. 


The Verification Officer is required to co-sign the form after sighting the relevant evidence. It is important that the Verification Officer does not store or retain the evidence provided by the RIW cardholder. 


Once all parties have completed the COVID-19 declaration form, it should be uploaded as the only evidence against the COVID-19 Employer based competency, in accordance with worksite procedure.
